I read a piece in one of David Sklansky's books listing 8 common mistakes.

Folding when you should call/folding when you should raise
Calling when you should fold/calling when you should raise
Raising when you should fold/raising when you should call
Checking when you should bet/betting when you should check

I can think of a few more;

Turning on the computer when you should be down the pub

Coming home from the pub and turning on the computer

Thinking I'll just pay the blinds one more time and then come off

Planning to have a good session on the table with no suck outs
